Clinical Director

DR SAMIR KHAN

As Clinical Director, I am responsible for the delivery of the PCN contract, and work with the PCN Board who represent each of our GP Practices to help guide strategic development and growth of the Network. I have oversight of the various workstreams that are geared towards delivering targeted healthcare services to our patient population and enhance the strength of our GP Practices. Our network is built from the ground up with our superb workforce guiding us towards providing excellent care. Part of my job is to help encourage their innovative thinking and foster the positive culture that we are creating together.

PCN Management

MAE FULLER

As the PCN Development Manager, I support the Primary Care Network to develop and implement service improvements and transformation work aligned with local and national priorities. I oversee all projects run by the PCN which includes COVID-19 vaccinations, funding and contracts. I act as a link to the ICS, NHSE and other external organisations to run and improve the services we can offer our patients. I work alongside the management team to make sure the PCN runs efficiently.

LISA JELLEY

As Clinical Operations Lead, I have clinical oversight of the Network DES (our PCN contract) and the IIF providing clinical guidance for Mae with the delivery of her projects, working with Danny and the CP team to support their work streams and leading/working with the CCOs to deliver the many innovative and forward-thinking ideas that form on the back of the DES & IIF. It is an absolute pleasure and privilege to be part of this PCN.

DANNY BARTLETT

I'm the lead pharmacist for the PCN. This means I manage and help the pharmacy team (made up of pharmacists and pharmacy technicians) to work effectively across all sites and make sure that the PCN is practicing good medicines safety. I also help the PCN in developing each practices' understanding and use of our amazing pharmacy professional workforce.

LUKE STEWART

I am the lead for the Social Prescribing and Health and Wellbeing Coach services in the PCN. I also work as a Social Prescriber. In my leadership role, I manage the team of Social Prescribers and Health and Wellbeing Coaches, who support patients to improve their wellbeing. I am constantly working with the team on ways that these new roles in primary care can develop and provide excellent care for our patients.

LUKE STEWART

I am the lead for the Social Prescribing and Health and Wellbeing Coach services in the PCN. I also work as a Social Prescriber. In my leadership role, I manage the team of Social Prescribers and Health and Wellbeing Coaches, who support patients to improve their wellbeing. I am constantly working with the team on ways that these new roles in primary care can develop and provide excellent care for our patients.

KIM TRICKEY

My role is to bridge the social care gap between our GPs and local services to help connect people with community organisations outside of the GP practice. This helps them feel more engaged and empowered to support themselves with their long-term illnesses and the difficulties they are facing.

Health & Wellbeing Coach

SALLY MILNE

Working as a Health & Wellbeing Coach at Horsham Central PCN, my role is supporting people to gain the knowledge, skills and confidence to make healthy, sustainable lifestyle changes.  Focusing on improving health related outcomes, working with people to set personalised goals and change behaviours so that they feel more in control of their health and wellbeing.  I became a certified health coach last year after twenty years working as a nutritionist specialising in women’s health and fertility.  I remain passionate about empowering people to have the confidence to take proactive steps to improve and maintain their own health and wellbeing."

Clinical Pharmacists

SAMIEAH KHAN

Hi I’m Samieah your local Clinical Pharmacist. Do you often have medication queries and struggle to find someone to discuss these with? This is where I can help you; I have the expertise to help answer all your medication queries including side effects, optimising your medications and ensuring you are getting the best out of your recommended treatment. I am based within your GP practice and easily accessible. I am here to help create an individualised care plan, putting you at the centre and focusing on what matters to you.

ISABEL MACHO

My role is to support you to manage different aspects of your health. Working together with patients and other medical professionals in developing treatment plans to monitor long term conditions . I am here to support on appropriate medicine dosages, medicine intake instructions, and other activities that would help patients achieve their healthy lifestyle and wellness.

ANILA PATEL

Hello, I am Anila and my role as a Clinical Pharmacist is to Optimise your medication treatment, advise on a better healthy lifestyle and create a better centre care of your Health and wellbeing via medication reviews- share decision making. I am here to listen, advise and sort out concerns on your health or medication. I work closely with our GPs and specialists to support you further.

Pharmacy Technician

AISTE GUOGYTE

We have good links with local community pharmacies, so if there are any stock issues, we work closely with them to solve the problem together. If you have recently come out of hospital, we are happy to have a chat with you about any new medications you may have.

NERINGA MILESKAITE

As a Pharmacy Technician I work alongside GP surgeries and Clinical Pharmacists to support the delivery of safe, effective and efficient systems and improve the quality of patient care. I handle clinical correspondence, inquires related to medicines, reduce medicine waste and maximise patient outcomes from their medication and much more. I also communicate with a wide range of Multidisciplinary team members to achieve positive impacts on patient’s wellbeing

Paramedics

SAM SMITH

As a paramedic working within primary care for the PCN home visiting team, Our role involves us undertaking assessments and gathering information for the GPs within the surgery. This is often done at a patient's own home if they are housebound or in clinic if they are not. We will gather relevant information in regards to your illness or injury, undertake advanced assessments to determine what is wrong, and then review this with a GP in the surgery in order to devise a plan of action or treatment. That could be medication, further testing or referral to a relevant specialist.

BECKY BURNETT

My role is to carry out predominantly housebound visits to assess, treat and diagnose patients. I regularly liaise with practice clinical staff to ensure the patient receives the best possible care. You may see me around each of the practices running in house clinics also.

MARY CROFFIE

My name is Mary and I am a Paramedic Practitioner and feel privileged to compliment and work alongside Becky, Sam, and Paul. I also conduct home visits and see patients with minor ailments in a clinic across the 4 surgeries within the PCN. I am currently undertaking Advanced Clinical Practice MSc Pg Dip at the University of Brighton and am a Non-Medical Independent Prescriber.

PAUL HARRIS

I'm Paul, a Paramedic for Horsham Central PCN. I split my time between here and SecAMB which allows me to have keep up to date with the continuous changes to practice. I work closely with Sam, Becky and Mary to provide support in the community, particularly to our housebound patients who require visits at home.

Physician Associates

UZMA AHMAD

As a Physician Associate in primary care, I work with GPs and the multidisciplinary team to diagnose and manage a variety of conditions from acute minor ailments to long-term chronic conditions. My generalist medical knowledge allows my role to be flexible and my day involves telephone triage, face-to-face appointments, interpreting investigations such as blood tests and referral to secondary care. I always have a GP supervisor on hand to ask for advice and discuss complex patients when needed.

EMILY GROSSWALD

As a newly qualified Physician Associate I work alongside the GPs and wider team to help diagnose and manage a wide range of conditions using my generalist medical knowledge. I work closely with highly supportive supervising GPs who I am able ask questions and discuss patients with as needed.
