Introduction

At Horsham Central PCN, our teams work incredibly hard to construct more accessible ways to help you manage your long-term health conditions. Our services may vary from seeing our Health & Wellbeing Coaches for holistic support to receiving an annual health review with one of our nurses to having a Clinical Pharmacist review and optimise your medication.

That being said, we are eager to support, empower and give you the tools to take charge of your health to improve your life whilst living with a long-term condition.

Diabetes

Preventing Type 2 Diabetes

Whilst type 1 diabetes cannot be prevented, type 2 diabetes is largely preventable through lifestyle changes. Around nine out of 10 people with diabetes have type 2 and there are currently two million people in England at high risk of developing type 2 diabetes, which is a leading cause of preventable sight loss in people of working age and is a major contributor to kidney failure, heart attack, and stroke. For people living with type 2 diabetes, the risk of dying in hospital with COVID-19 is also twice that of people who don’t have the condition.

 

The Healthier You

The Healthier You NHS Diabetes Prevention Programme, also known as the Healthier You programme, identifies people at risk of developing type 2 diabetes and refers them onto a nine-month, evidence-based lifestyle change programme. It is a joint service from NHS England and Diabetes UK.

The programme is open to people who are ged 18-79 (if you are over the age of 80 and would still like to be referred, please contact your GP), are not pregnant and do not have diabetes currently. Click here to find out more and assess your risk.
